Odan Healthcare Agency is a Domiciliary Care service set up to provide care for people in the comfort of their own homes. We aim to provide the support that people need to meet their day to day needs and to be able to make a difference.
Our service users are families who need support with looking after adults with complex care needs, genetic birth disorders, challenging behaviour and who may also have a combination of other needs. Our primary purpose is to provide personal care and support and to bring comfort to service users and their families. At Odan Healthcare Agency, we believe that when a vulnerable adult needs care and support, they want someone they can trust, someone who understands their needs, and someone whom families can rely on.
Service users with Multiple Conditions
Service users with multiple (two or more) chronic conditions whose conditions are expected to last at least 12 months or until the death of the service user.
Service Users in Complex Conditions
condition is so complex, that it places the individual at significant risk of death, acute exacerbation/decompensation or functional decline.
Individuals Requiring Clinical Staff Time
Individuals requiring 60 minutes of qualified clinical staff time, whose care is directed by a physician or other qualified care provider, per calendar month.
Creation of Comprehensive Care Plan
Establishment or substantial revision of a comprehensive care plan and who may require Moderate or high complexity medical decision-making
